Botulinum toxin is a naturally occurring protein which is produced by the bacterium Clostridium botulinum. This bacterium makes several different types of this protein which are called type A to type F. These varying proteins cause muscles to relax at different levels.
Currently two different types of botulinum toxin are commercially available in the United Kingdom:
Type A toxin: Botox (Allergan ), Dysport (Azzalure) and Xeomin (Bocouture)
Type B toxin: Neurobloc.
Botulinum toxin can be used to treat all wrinkles which are the result of normal day to day facial movements. When it is injected into a specific muscle in the face it blocks the signal from the nerve to the muscle. This means that the nerve ending cannot tell the muscle to move, hence there will be less wrinkling of the skin in the areas treated. I like the analogy of a piece of cardboard. If you fold it into a fan shape, the more you open and close the fan, the more wrinkling is apparent. The less times you do this the less pronounced the folds (wrinkles) in the cardboard are.
The upper face is where the best treatment results are obtained, commonly for the reduction in crow's feet (laughter lines) around the eyes and wrinkles on the forehead and between the eyebrows (frown lines).
Different names- same product?
There are several anti-wrinkle formulations on the market. It can all be very confusing so I am attempting to tease out the important bits of information below.
The most common preparations in the UK all contain Botulinum Toxin Type A.
People who have anti-wrinkle injections are likely to have had Botox, Bocouture or Azzalure used for treatment.
In March 2006, Allergan ®, the brand name used for Botox® with a dosing specific to treat glabellar lines was granted a licence in the UK from the Medicines and Healthcare products Regulatory Agency (MHRA) for the "temporary improvement in the appearance of moderate to severe glabellar lines (vertical 'frown' lines between the eyebrows) in adult women and men aged 65 and younger, when the severity of these lines has a psychological impact for the patient".
The same licence was granted to Dysport®, under the brand name Azzalure® in March 2009 and to Xeomin® under the brand name Bocouture® in July 2010.
If you are considering having a botulinum toxin treatment in this country, the above is nothing to be concerned about. All U.K. practitioners can use any of the below brands:
BOCOUTURE®, botulinum toxin type A, has been available in the U.K. since 2008 as the brand name Xeomin®
Azzalure, botulinum toxin type A, available in the U.K. since the early 1990's as Dysport and is approved for cosmetic use in the UK to treat glabellar (frown) lines.
Dysport, botulinum toxin type A, available in the U.K. since the early 1990's and has licences in 67 countries worldwide including the United Kingdom and also in many European markets
Neurobloc ® recently available in the UK since 2001. There is limited experience in the use of this type of toxin, and the product does not currently have approval for cosmetic use anywhere in the world.
With dosing specific to treat glabellar (frown) lines, Botox, botulinum toxin is also marketed for cosmetic use in the UK under the brand name Allergan .
Xeomin® is the third botulinum toxin type A licensed in the UK. It is an innovative Botulinum type A formulation, in which the complexing proteins have been removed
So how do you know which one is right for you?
Botox is the most famous brand, it has more proteins and is more like the natural product but possibly could stimulate a greater immune response compared to the other products. It is the product closest the naturally occurring form.
Azzalure and Bocouture have fewer proteins, with Bocouture containing the least. This makes the product more “pure” but it may not contain all the properties conferred by Botox
